COMPASS & SONYC APPLICATION

United Activities Unlimited’s afterschool programs are funded by the New York City Department of Youth and Community Development (DYCD). Elementary programs operate under COMPASS, the Comprehensive After School System of New York City, serving grades K through 5. Middle school programs operate under SONYC, School’s Out New York City, serving grades 6 through 8. DYCD covers the cost, so families are charged no tuition.

Programs begin at the host school’s dismissal bell and run Monday through Friday. Hours and activities vary by site.

How To Apply

A

Complete the DYCD application.

Every child enrolled in a DYCD-funded program must have a DYCD application on file. This is a DYCD requirement, and UAU cannot enroll a child without it. Go to DiscoverDYCD at discoverdycd.dycdconnect.nyc and create an account or sign in if you already have one. Select After School Programs, search for United Activities Unlimited, and choose the site your child will attend. Add the program to your list, return to your dashboard, and submit the application. If you would rather complete a paper version your Program Director.

Complete the UAU Registration Form Below.

Fill in every required field and upload your child’s medical form. The medical form must be dated within the last 12 months. An expired medical form, or a missing one, leaves the application incomplete, and your child cannot be enrolled until a current one is on file. Have the form ready before you start. If you do not have it on hand, you will need to return to this page and register once you do. You can download a blank CH-205 Child and Adolescent Health Examination Form by CLICKING HERE

Submitting this form below does not enroll your child, and it does not hold a seat. Each site has a fixed number of seats set by DYCD. We fill them on a first-come, first-served basis, in the order that completed applications arrive.
